Option 1
Public Assistance Recipients
A full copy of an award letter (dated within the last 12 months) for HEAP, SNAP, TANF, or Supplemental Security Income (SSI).

Employment or Other Income
- Pay stubs for all household income from the last 60 days
- Social Security/Disability award letter
- Documentation for pensions, unemployment, Veteran’s benefits, annuities, or other income
- Self-employed? Provide your IRS quarterly earnings report for the last three months

Tax Return Verification
Provide most recent Federal Income Tax Returns (if all household members required to file have done so).
